R+V Health Insurance's bKV Plan: Rates, Benefits, and Costs

The PROFIL health insurance plan from R+V Krankenversicherung is structured as a budget-based plan: Instead of individual benefits, employees receive an annual reimbursement limit that covers various outpatient and preventive care services. Employers choose one of five budget tiers ranging from 300 to 1,500 euros per year, with a total of seven plan options available, including combinable modules. This structure allows employers to tailor the level of coverage specifically to the desired budget per employee without having to purchase a separate plan for each individual service.

Numerous outpatient services are reimbursed within the health care budget: preventive checkups beyond those covered by the statutory health insurance (GKV) catalog, travel vaccinations and routine immunizations, treatments by alternative practitioners in accordance with the applicable fee schedule, naturopathic treatments—including osteopathy—provided by physicians or alternative practitioners, as well as medications, bandages, therapeutic aids, and medical devices. Professional dental cleanings, as a preventive service, are also included. A separate sublimit applies to vision aids within the budget, meaning that the full annual amount is not available for this purpose. As a general rule, statutory health insurance requires that claims under the statutory health insurance (GKV) be exhausted first before the health budget takes effect.

Optional inpatient services, such as single or double rooms, are not included in the base plan itself; they can be added through the separate modules R+V-Klinik 1 PROFIL and R+V-Klinik 2 PROFIL, which provide private medical care. A daily hospital allowance is also not included in the health budget. Dental prosthetics are not covered by the current budget but rather through the standalone Dental PROFIL module, which is available in tiers of 900 or 1,800 euros and can therefore be combined independently of the selected budget tier of the health budget. In addition, digital health services are available, such as a specialist appointment service, a medical hotline, and the option to obtain a second medical opinion.

Employers with a minimum of five employees are eligible for this program. This means it is also aimed at smaller businesses that would like to introduce a supplementary health insurance plan without needing to have a large workforce.

Family members can be covered under appropriate plans, making family coverage an integral part of the program. If an employee leaves the company, they have the right to continue coverage: The PROFIL health budget can be continued as individual insurance without a new medical examination, provided the appropriate declaration is submitted within two months. This facilitates a seamless transition to continued private insurance coverage.

Thanks to its low minimum company size requirement and tiered budgeting structure, the PROFIL health budget plan is suitable for both small and medium-sized businesses that want to tailor their supplemental health insurance flexibly based on their budget and desired scope of coverage. Costs

Contributions to the PROFIL health budget are based on the selected budget tier: Employers set an annual budget per employee ranging from 300 to 1,500 euros, and the contribution increases with the level of that tier. This allows the level of benefits to be managed consistently across the entire company without the need to calculate costs separately for individual types of benefits.

The outpatient services included in the budget—such as preventive checkups, treatments by alternative practitioners, naturopathic therapies, medications, and medical aids, as well as professional dental cleanings—are deducted from the health budget. A separate sublimit within the overall budget applies to vision aids. Services covered by statutory health insurance must always be used first; only the remaining balance is reimbursed from the health budget.

The budget is set on a calendar-year basis and does not include a carryover provision. Unused amounts expire at the end of the year and are no longer available in the following year. Additional coverage options, such as Dental PROFIL for dental prosthetics or the hospital coverage options for elective inpatient services, operate independently of the health budget and are calculated separately.
