Signal Iduna's Supplementary Health Insurance: Plans, Benefits, and Costs

Signal Iduna organizes its offerings for employer-sponsored health insurance into three plan structures: GesundAGIL+, GesundAKTIV+, and GesundVITAL+. All three are structured as budget-based plans, in which the employer sets an annual health budget per employee ranging from 300 to 1,500 euros, divided into five tiers. The combination of the three plan structures with the five budget tiers results in a total of 15 plan options, allowing companies to tailor coverage precisely to their workforce and budget without being locked into fixed benefit packages.

Within the selected budget, outpatient services are reimbursed without requiring prior payment by the statutory health insurance provider. This simplifies billing, as employees can submit invoices directly. Coverage includes, among other things, 100 percent reimbursement for preventive medical checkups, services provided by alternative practitioners, and naturopathic treatments, as well as osteopathy and naturopathically prescribed medications and bandages. For medications, only the copayment is covered, while medical aids are reimbursable, including the copayment. Although vision aids are included, they are subject to a sublimit within the health budget, meaning that only a limited portion of the annual budget is available for this purpose.

The budget-based approach also pays off when it comes to dental care: Professional dental cleanings and teeth whitening are included as benefits and are funded from the respective health budget. Preventive care is further supported by protective and travel vaccinations, which are also included in the scope of coverage. The plan does not include a separate daily hospital allowance. The insurance coverage is supplemented by digital health services such as video consultations via MD Medicus, a specialist appointment service, a medical hotline, and a second medical opinion, which make it easier for employees to access medical advice without having to visit a doctor’s office.

From an organizational standpoint, the barrier to entry is low for small businesses: GesundAGIL+ and GesundAKTIV+ are available to companies with as few as three employees, while GesundVITAL+ is only available to companies with a workforce of at least 20 employees and is therefore geared more toward larger companies seeking a more comprehensive set of benefits.

Employees can include their family members in their Signal Iduna supplementary health insurance (bKV) policy. If an employee leaves the company, insurance coverage does not automatically end: The policy can be continued privately under the budget plan, ensuring that the coverage established remains in effect even after the employment relationship ends.

Thanks to the low minimum size requirement for two of the three benefit plans, this offering is suitable for both small businesses and larger companies, which can access a more comprehensive benefit plan through GesundVITAL+. The quality of the plan is underscored by several external ratings: Franke & Bornberg rated it “Outstanding” in 2025, Morgen & Morgen awarded it the top rating of “Excellent” (5/5) in the same year, and the IVFP rated the plan “Very Good” in 2024.

Costs

The premium amount for Signal Iduna’s supplementary health insurance (bKV) depends largely on the selected budget tier. Since all three plan options are tiered with annual budgets ranging from 300 to 1,500 euros per employee, the employer’s choice directly determines the financial scope of the insurance coverage and, consequently, the cost basis for the company.

Within the selected coverage level, eligible outpatient and dental services count toward the annual budget. This applies, for example, to services provided by alternative practitioners, naturopathic treatments, osteopathy, as well as professional teeth cleaning and teeth whitening. Certain types of services, such as vision aids, are additionally subject to a sublimit within the overall budget, meaning that only a portion of the annual budget is available for them, even if the overall budget has not yet been exhausted.

Since no advance payment from the statutory health insurance is required, submitted invoices are paid directly from the agreed-upon budget. Choosing a higher budget tier correspondingly expands the reimbursement limits for outpatient, dental, and preventive care services, while a lower tier limits costs for the employer but also reduces the reimbursement limits for employees.
